If you are going to partake in this treatment, please read the information below and follow the requirements:

1. Before treatment, please make sure all areas of the skin are clean
2. No coffee or alcohol 24 hours before treatment
3. Make sure all clothing around area is comfortable and loose. Any tight clothing in procedure area can cause irritation
4. Be aware a few days of downtime may be required for swelling to subside
5. No perfume, deodorants or any other skin products the day of treatment
6. We recommend any client getting a neck or stomach procedure go to their doctor and get a lidocaine 20% prior to treatment to reduce sensitivity to areas
7. Please give us 48 hours notice prior to treatment if you need to cancel
8. Deposit is non-refundable. If late cancellation, this deposit will go towards our loss of time and loss of filling your appointment with another client.

Before any procedure can commence you must fill out the two forms below the price chart. The Disclosure and Consent form and the Medical History form, must be deliverd to us prior to the procedure. Please contact us for more information about appointment times and when these forms must be back to us. Without the forms and allowable time to go through them, no procedure will commence under any circumstances. This is for your safety as well as ours.
